ASSESSMENT OF NEED:

We will process your form and book a call with you to request some further information. Based on the outcome of this initial stage, we will then do a fully comprehensive assessment.​​

 

2.

MDT DISCUSSION:

The findings and reports from the assessment and initial referral will be discussed with our Multi-Disciplinary Team.

 

3.

INDIVIDUAL VISITS THE PROPOSED LOCATION :

We will arrange for the referred individual to be able to visit the proposed placement (unless it is in their own home) so that we can get their input in regards to customisations

 

4.

CARE COORDINATOR MEETS WITH COMMISSIONERS:

Our Care Coordinator will meet with the commissioners in order to discuss the proposed package.

 

5.

FUNDING AGREED:

We will then wait for funding to be agreed by the appropriate parties.

 

6.

DEVELOP CARE PLAN:

Based on the outcome of the above steps, along with all the other information gathered in the process, we will create a care plan as well as an activity plan, personalised life skills plan and a Positive Behaviour Support Plan.

 

7.

TRANSITION VISITS:

The visits for the transition period are arranged in order to create a smooth transition from the current placement or care provision arrangements to becoming part of The Ancora Family.

 

8.

MOVE-IN DATE SET:

The Date is set for the move to take place and all the customisations, specific staff training, system onboarding and other preparations take place in a timely fashion leading up to the date.
